Why Certificate Format Matters
The format of a medical certificate determines its legal acceptance. Employers, schools, and insurance companies have specific requirements for the information that must appear on the certificate. A certificate missing any key field may be rejected.

Every numbered field serves a specific purpose:
- [1-2] Hospital identity — establishes authority and contact for verification
- [3] Certificate type — sick leave, fitness, school, etc.
- [5-7] Patient identity — must match official ID documents
- [8] Visit date — confirms examination actually took place
- [9] Diagnosis — can be specific or general (patient may request general)
- [10-11] Leave period — legally defines the approved absence
- [14-17] Doctor credentials — allow verification of certificate authenticity
